Success Rates for IVF at Fertility Argentina

The key of our success rate
I work in a very large egg donor program (http://www.fertilityargentina.com/in-vitro-argentina/egg-donation-in-argentina/). I take care of patients coming from all around the world: specially from USA, Canada, Australia, New Zealand and some countries of Europe and all South America. Why do all these patient fly overseas? Why do they decide to come to Argentina instead of going to other countries? Why is our program the leader?
Although I could write long explaining all the reasons, this time I will focus on our success rate and the guarantees that we give (next times I will talk about costs, tests done to the donors, the short waiting list and the simplicity of the whole process…you can read about the last seminars that I gave abroad here: http://www.fertilityargentina.com/dr-glujovsky-australia-4-days-brisbane-hobart-talking-patients/).
Success rates and guarantees
In our egg donor program we guarantee that all our patients receive at least 6 mature eggs. Most of them receive more than six. In fact, the average is 8-9. In the past, we published a paper showing our experience in shared egg donor programs (we were one of the pioneers on this), and we showed that receiving 4 or 5 eggs was good enough but, we also showed that when receiving 6 or more, success rate was higher and the freezing rate was increased too. And that is why we decided to guarantee at least 6.
Once the eggs are fertilized (we do ICSI to all of them) using own or donor sperm (according to the needs of the patient/s), we wait 5 days until we do the embryo transfer. We transfer the embryos only in blastocyst stage. We guarantee that all our patients have at least one blastocyst to transfer. What happens if, for some reason, there are no blastocysts to transfer? In that case, we pick another donor, with no additional cost, and we do another egg donor cycle again to finally have at least one blastocyst to transfer. 85% of our patients have more blastocysts and freeze some of them.
Why do we put so much emphasis to transfer on blastocyst stage? Because there is much evidence supporting the transfer on blastocyst stage. It is known that transferring on blastocyst stage is more effective (higher live birth rates) than doing it on day 3. Last year, I published a Cochrane systematic review showing all these data and then another paper at Fertility and Sterility confirming this (http://www.fertilityargentina.com/blastocyst-versus-day-3-embryo-transfer-systematic-review/).
In summary, one the keys of our success rates in egg donation are: guarantee of at least 6 mature eggs and guarantee of blastocyst transfer…in a great embryo lab. Next time, I will talk further about some other reasons why international patients come to Fertility Argentina for egg donation.

IVF Cost at Fertility Argentina – Egg Donor Program

Here is some information from Fertility Argentina regarding their egg donor program:

I work in a large Egg Donor Program in Buenos Aires, Argentina (with ISO 9001 certification of quality). Every year, more than 550 women/couples receive donor eggs and more than half of them get pregnant in each cycle. Our clinical pregnancy rates are 60-65%. We are experienced at coordinating the egg donor process from abroad; about 20% of our couples come from outside Argentina, mainly from USA, Canada, Australia and New Zealand.
Our objective is the same as yours: to get a take-home baby as soon and as simple as possible, with the highest safety (infectious and genetics) parameters. We will analyze your case and will build a customized plan for you. The difference with others is not only based on what we do but, also, and not less important, on how we do it. Our criteria is: highest pregnancy rates, only one trip to Buenos Aires, and without waiting list, in a program with cosmopolitan donors and with a direct patient-to-doctor communication.

We have a program based on sharing donated oocytes. It means that one donor donates her oocyte cohort to several recipients. Each recipient gets a minimum of 6 mature eggs (MII oocytes), although the average is 8-9. We do ICSI to all of them. We guarantee to transfer at least one embryo in blastocyst stage (and 85% have more than one blastocyst and can freeze them for a second attempt). Sharing the oocytes this way, allows us to reduce costs and shorten the waiting time, while continuing to have very strong pregnancy rates.

Egg donation is anonymous. However, we will try to match the donor with the recipient as much as possible. Total costs are the following:

  • First office visit: USD 400 (American dollars)
  • Egg donor cycle: USD 6,900 (American dollars)

Donor’s medication is included. Recipient’s medication is not included (but it is not expensive). Payment is cash or with a bank wire 1-2 weeks before coming. Embryo freezing item will be paid cash (USD 1000) and will be refunded in case that there are no embryos to be frozen.

Patients usually stay in Buenos Aires for about 10 days.

1. What is the current success rate for your clinic? (preferably by age as I know that the success rate will be much lower for somebody of my age) IVF success is like a multi variable equation. Age by itself makes up for only one variable and there are many other factors such as the female patient’s hormone profile, the male patient’s semen characteristics, uterine conditions and many more. However, assuming that there are no major problems with most of these factors, in other words, holding everything constant, treating age as the only variable, IVF success in the 40-44 age bracket is around 19%. However, our clinic’s cytoplasmic transfer procedure has contributed greatly to our success rates with patients in your age bracket. Although this is a relatively new procedure and we would like to have more than 200 cycles before publishing statistics, I can say that cytoplasmic transfer has a success around 24% in this age bracket.

2. How many cycles do you perform per year at the clinic? We perform around 550 cycles per year.

3. What is the maximum age that you treat? We treat up to 50 years of age.

4. Are there any fertility conditions that you will not treat? Not specifically. However, we have a very thorough screening and testing process before we make an assessment of your unique fertility needs and requirements. If we believe that there are no prospects for success with a certain treatment, then we will recommend against it and will not assist with such a treatment (For instance, a menopausal patient requesting treatment with own eggs will not be treated as the outcome is more than likely to be a failure).

5. What is your embryo transfer policy? How many embryos will you replace at one time? We are able to transfer up to three (3) embryos legally.

6. What is the basic cost of the IVF treatment? IVF + ICSI treatment costs 2,250 Euros for the treatment itself. Other treatments will have different cost of treatment. For instance, IVF with Cytoplasmic Transfer costs 5,000 Euros but it involves using an egg donor since we will need to use the cytoplasm from a young and fertile egg donor to provide the patient’s genetic material a more friendly environment to grow.

7. What additional costs would / could be incurred? Additional cost of treatment will include a)medication b)flights c)accommodation d)pre-testing prior to treatment. Cost of medication depends on what dose of medication is administered, which medication protocol is used and where the medication is obtained. Pre-testing involves hormone testing, semen analysis, and any other testing that is seen fit by our IVF specialists before we can make an assessment and decide on further course of action with your treatment. It is important that you are thoroughly investigated before treatment so that we can maximize the chances of success with your first attempt and reduce the likelihood of needing another testing and treatment process.

8. What happens to the costs if the cycle has to be abandoned? We do not require any payment if a cycle is abandoned before you travel to Cyprus for your treatment. We can organize so that your preliminary work is done locally under our guidance and you can come to Cyprus for less than a week to finalize your treatment. If your initial screening indicates that we are not receiving desired response with your ovarian stimulation, then we can cancel the cycle. That way, you don’t have to travel to Cyprus and you don’t make any payments to our clinic. We do offer risk-free treatments in this sense. However, if you arrive in Cyprus, all the procedures are performed and for some reason there are no embryos to transfer, then the cycle will be canceled, however, we will ask for a payment for the work that has been done up to that point.

9. How long would I need to be in Cyprus for? You are likely to stay in Cyprus for 5/6 days. You will also have the option of coming here for the entire treatment if it is more convenient for you to have it all done at one place. This will require you to stay for 21 days.

10. Who deals with the logistics of flights and accommodation? We do not deal with flight bookings, however, we do assist with accommodation. Our patients can stay at discounted rates at a number of collaborating hotels in Cyprus. We also offer complimentary transfers to and from Ercan and Larnaca Airports for all our patients.

11. How is the pre-treatment carried out? We do collaborate with a number of doctors, testing centers and pharmacies in Europe and the USA. That way, we can have your initial testing and screening performed locally. Once we receive your test results and other related documents, our specialists can make an assessment of your fertility treatment needs and we can organize your treatment protocol accordingly.

12. How do I get medication? We work with an international pharmacy in the UK which delivers worldwide. Therefore, if your local monitoring doctor is not willing to accept international orders, then we can organize your medication delivery through the UK pharmacy.

13. How will I be monitored? Your treatment calendar will let you know exactly what needs to be done on a daily basis and how many times you will need a scan. If you do not have your own gynecologist who can monitor the ovarian stimulation stage, then we will recommend a name depending on your location. The places that we work with are able to perform scans and required tests and they communicate results within the same day, so we are kept up to date with everything related to your initial monitoring.

14. How will I know that I am not being under / over stimulated? If your scans indicate less than optimal or more than optimal response, then we will be able to reassess your medication use accordingly. This is precisely why you are being monitored locally before traveling to Cyprus. However, your medication dose is carefully identified in the light of your hormone levels, prior IVF treatment outcomes (if any), your age and your baseline ultrasound scan view, therefore, it is not likely that there will be an issue of under/over stimulation.

15. If I have a problem whilst in the UK who would I go to? An IVF specialist that we collaborate with.

16. Is the clinic registered with any regulatory body? Yes, local government accreditation is in place as well as ISO certification.

We do not have a formal age limit but will assess each case individually and give an opinion on the likelihood of success.  We believe strongly that patients need to be given honest and comprehensive information about treatment options.

In the UAE, we are unable to offer treatment involving the use of donated sperm or eggs

 

5.    What is your embryo transfer policy? How many embryos will you replace at one time?

 

Our maximum limit is usually two embryos as there is a substantial amount of evidence showing that this gives the best live birth rate, as opposed to the best pregnancy rate. If more embryos are transferred, multiple pregnancy is more likely and this leads to a higher risk of miscarriage.

 

6.    What is the basic cost of the IVF treatment?

 

We offer a number of treatment packages but the basic cost would be AED28,000 without medication.

 

7.    What additional costs would / could be incurred?

 

Medication can add substantially to this, maybe AED5000-8000 depending on circumstances. Some patients may require certain tests done, such as hormone blood tests. We do offer a number of inclusive packages that can be tailored to individual needs.

 

8.    What happens to the costs if the cycle has to be abandoned?

 

If a cycle is abandoned before egg collection, the patient will only have to pay for the medication used.

 

 

9.    How long would I need to be in Dubai for?

 

This depends on the exact regime used but, in general, treatment can start with a period and be completed by about 3 weeks later.

 

10.  Who deals with the logistics of flights and accommodation?

 

Our administrative staff can assist with travel and accommodation requirements.

Pre-treatment may not be necessary in  most cases. It is best if the patient can be seen at some point before treatment to check for major issues but this can be done locally with their regular physician.

We conduct regular ultrasound scans and blood tests throughout the treatment cycle, so several visits are required.

15.  If I have a problem whilst in the UK who would I go to?

 

We do have a clinic in the UK, near Cambridge , but obviously this will be convenient for everyone, in which case a local gynaecologist can usually help. We are happy to liaise with them as needed.

As required, the clinic is registered with the Dubai health Authority and licensed for IVF treatment. This is the only regulatory requirement in the UAE.

9. How long would I need to be in Dubai for? The treatment lasts for 3-4 weeks.

10. Who deals with the logistics of flights and accommodation? We have a page on our website to help international patients find a hotel. We don’t take care of flights, but we can issue a letter of invitation if it helps.

11. How is the pre-treatment carried out? Depending on the case, we sometimes start stimulation outside the UAE. Usually we do that for couples who prefer to remain in their country and who don’t have major fertility problems, or couples with a male infertility problem and the wife is healthy). All tests and stimulation will be liaised between our clinic and an obstetrician/gynecologist in their country. Also, some men who need to come to us for Micro-TESE operation and who send us their hormone results may need to take some supplements or medication to help them produce sperm so that when we perform the operation there is a higher chance of finding sperm.

12. How do I get medication? Through your doctor, and based on our recommended doses which are based on the results of blood tests and ultrasounds.

13. How will I be monitored? Through regular blood tests and ultrasounds.

14. How will I know that I am not being under / over stimulated? We closely monitor your hormone levels by regular blood tests and ultrasounds.

Gender Selection Cyprus for Affordable Family Balancing

Gender Selection is offered in a few alternative methods at North Cyprus IVF:

1) MicroSort IUI for Gender Selection

This treatment method is very much like conventional IUI apart from the fact that male partner’s sperm is sorted into X and Y bearing cells through the MicroSort procedure. The MicroSorted sperm is then used for artificial insemination where it is injected intra-uterine.

MicroSort for gender selection with IUI treatment has 90% accuracy for gender selection for a girl and 75% accuracy for gender selection for a boy. Regarding success rates, MicroSort IUI has similar success rates to IUI treatment success rates, which is about 20-25% for women younger than 35 years of age, and about 8-10% for women older than 35 years of age. It should be kept in mind that MicroSort IUI may need several attempts before pregnancy can be achieved.

2) MicroSort IVF for Gender Selection

This treatment option is more like regular IVF-ICSI treatment. The male partner’s sperm is sorted into X and Y bearing cells for gender selection, just like in the IUI option. However, as opposed to injecting the sorted sperm trans-uterine, female partner’s eggs are taken out and the sperm is injected directly into the nucleus of the egg for fertilization. In other words, IVF+ICSI does not let nature take its course, but rather, does the job in the lab environment.

IVF treatment requires that the female partner uses stimulation medication in order to produce a higher count and quality of eggs in a given menstrual cycle. It is through this medication protocol that we are able to obtain a higher count of eggs in order to increase your chances of pregnancy.

Accuracy for gender selection is still around 75% for a boy and 90% for a girl. However, pregnancy rates are considerably higher than IUI since IVF procedures allow for multiple eggs to be fertilized and transferred back into the uterus.

3) IVF with PGD for Gender Selection

Alternatively, there is PGD which is a genetic testing procedure done once the embryos are created. It allows us to identify the gender with 99.9% accuracy as well as screening for most common genetic disorders, making sure your baby does not possess a serious genetic condition. This method is a bit more expensive compared to MicroSort IVF but is more accurate. Gender selection through PGD works just like conventional IVF, where eggs and sperm are fertilized in the laboratory environment. Then, embryos are placed in an incubator for optimal growth. Once the embryos reach the 8-cell growth stage (usually day 3 after fertilization), biopsy is performed where a single cell is retrieved from each embryo for diagnosis. This biopsy allows us to test for certain genetic conditions and determine the sex. Apart from the sex of the baby, we test chromosomes X, Y, 13, 18 and 21, which are the causes of almost 90% of all genetic-linked abnormalities. However, in this option, the specimen is used without MicroSort.
